Peer Counselor Level II (CCPEP Bridger)(Two Positions)

Purpose of Position:

The Peer Counselor, under the supervision and in collaboration with staff, will serve as a family peer advocate, planning and implementing treatment plans for patients and families going through mental health emergencies and connecting them to ongoing care and support. Provide direct services to patients and families in an emergency setting and after discharge. Empower peers to advocate for themselves, resolve issues or problems and plan for their futures by serving as a role model.

Job Responsibilities:

– With the supervision of a social worker, providing timely and complete assessment of social factors that impact on all patients assigned and planning appropriate interventions.

– Providing concrete supporting services to patients and families.

– Providing information about community resources and appropriate aftercare referrals.

– Escorting patients into the community and making home visits as needed.

– Collaborating with the multidisciplinary team.

– Working closely with related social and community agencies.

– Participating in Interdisciplinary Performance Improvement Projects.

– Required to do electronic documentation.

– Other tasks as assigned by Supervisor and/or designee.
