Peer Specialist

The Peer Specialist will provide non-clinical support and advocacy services to individuals within the Integrated Clinic through the appropriate disclosure of their lived experience. The Peer Specialist will work as a member of the multi-disciplinary team and provide services as documented on the treatment/recovery plan.
Responsibilities
- Demonstrate understanding of the peer role. Utilize lived experience to engage people from various settings into treatment. Provide support, encouragement, and guidance in linking individuals to appropriate services.
- Maintain confidentiality and appropriate boundaries with individuals.
- Follow-up on treatment recommendations and referrals made by clinical staff and assist with adherence to treatment appointments. Provide warm handoffs to treatment providers and/or recovery support services and other community resources as indicated in the treatment plan.
- Provide outreach, advocacy, mentoring and recovery support services to those seeking or sustaining recovery.
- Participate in on-site and off-site activities with participants.
- Responsible for crisis intervention and incident reporting procedures
- Act as an advocate and liaison for participants for access to supports.
- Works with Emergency Department staff and Behavioral Health team to identify community support services
- Collect any necessary data to assist in reporting to program, agency, and/or funding sources. Document services in program EHR following requirements set by OMH, OASAS, SAMHSA and Medicaid.
- Assume other responsibilities and tasks as assigned
Qualifications
- H.S Diploma or Equivalent
- Completion of Core Courses through the Academy of Peer Services
- New York Certified Peer Specialist-Provisional (NYCPS-P) eligible
- Relevant life experience required which includes personal experience living with a disability, history of mental health diagnosis, or social/emotional challenge, and willingness to draw upon this experience to serve as a role model to support the recovery goals of individuals served.
- Familiarity with recovery skills, support services, and local community resources required.
- Valid NYS Driver’s License required.

Salary: $22.64/hr
