JOB DEFINITION:

 

The OnTrackNY Program is a Coordinated Specialty Care (CSC) treatment program at CN Guidance and Counseling Services, which serves adolescents, and young adults who are experiencing first episode psychosis. Our mission is to empower young people to make meaning of their experiences and to pursue their goals for school, work and personal relationships. Equity, inclusion, rapid access and self-determination are at the core of everything we do.

 

The Peer Specialist will work as part of the OnTrackNY team with program participants, family and community members, and all other team members. The Peer Specialist serves a unique role on the team, instilling hope in participants and families by drawing on their own lived experience of overcoming challenges with a psychiatric label. Walking alongside participants on their recovery journeys, the Peer Specialist uses a non-clinical approach to support, empower, and advocate for young people as they contend with the difficulties and uncertainties of their emerging experiences.

 

EDUCATION REQUIREMENT:

 

At a minimum a peer must, have a high school diploma, high school equivalency or a State Education Commencement Credential and NYS certification as a Peer Specialist or an ability to obtain certification within one year of hire, (CERPA-P).

 

EXPERIENCE REQUIRED:

 

A person aged 18- 30 years old who has lived experience with mental health and/or co- occurring behavioral health challenges in their home, school and/or community

 

DUTIES AND RESPONSIBILITIES:

 

  • Outreach & Engagement: Connecting with both the community and participants and families about OnTrackNY and offering hope and support around the possibility of

 

  • Relationship Building: Forming meaningful partnerships with program

 

  • Embracing Creative Narratives: Expanding the understandings of the experience of “First Episode Psychosis”.

 

  • Co-Creating Tools for Success: Partnering with participants and families to support development and creation of self-care and self-awareness tools.

 

  • Supporting and Partnering with Families: Working closely with a participant’s family of

 

  • Making OnTrackNY Better: Supporting the growth and development of OnTrackNY through creation and support of a feedback loop between participants and the

 

  • Identify and implement quality improvement trainings and activities as part of professional development goals.

 

  • Encourage quality improvement processes and provide consistent resources for QI and professional development.

 

  • Ability to cultivate a culture of inclusion for all employees that respects their individual strengths, views and experiences. A Culture that makes better decisions, drives innovation, and delivers better business results.
  • Adherence to all safety protocols and procedures to ensure a safe working environment for all employees. Demonstrate a commitment to maintaining a safe work environment by following established safety guidelines.
  • Performs other duties assigned.
