CORE Peer Support Specialist

Job Requirements:
- Responsible for delivering trauma-informed, non-clinical services with a focus on recovery and rehabilitation from a behavioral health disorder
- Facilitate engagement with participants by developing genuine relationships with participants and their supports by using empathy, sharing experiences, motivational interviewing, active listening, and collaborating on person-centered goal-development
- Educate participants and family members on the domains of person-centered wellness and recovery
- Identify community resources and encourage help-seeking behaviors
- Participate in on-site and off-site activities with participants
- Responsible for crisis intervention and incident reporting procedures
- Act as a role model and coach for participants
- Provide services to participants to address housing, income support, education and vocational training, social supports, employment, recovery supports, primary care needs and psychiatric rehabilitation programs
- Assist participants with identifying and sustaining coping techniques and self-help strategies
- Provide individual peer support and wellness counseling
- Act as an advocate and liaison for participants for access to supports
- Escort participants as needed to various appointments via Public Transportation
- Conduct follow-up via telephone outreach, visits and or certified letters
- Ensure documentation are completed within electronic medical record in a timely fashion
- File various information into the client’s individual record
- Attend staff meetings and other meetings as directed by supervisor(s)
- Participate in OMH/ NHCC approved Orientations and Training
- This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor
- Qualifications/Requirements:
- High school diploma or equivalency diploma required
- Currently certified by the New York Peer Specialist Certification Board or NY Certification Board (CRPA)
- Ability to work under strict confidentiality with an ability to be receptive of feedback and supervision for growth and development
- Proficient in Microsoft Word and Outlook
- Excellent writing, editing and verbal communication skills
- Bi-lingual in Spanish a plus
- Excellent interpersonal skills with ability to interact culturally, linguistically, and diplomatically
- Travel Requirements – Ability to travel throughout designated county
